Description

Seeking a Community Liaison (sales rep.) for home health/home care with a great company, that's well established in the community and trusted for their excellent patient care. This business development representative, will drive patient referrals by building relationships with hospitals, physicians, and skilled nursing facilities. Educating providers, patients, and families on care options, manage community marketing, and ensure smooth patient transitions to care.

Key Responsibilities

Business Development: Establish and maintain relationships with referral sources, including case managers, social workers, and physicians to drive growth.

Community Outreach: Conduct in-person meetings, deliver presentations (lunch & learns), and attend community events to increase company visibility.

Patient Transition/Education: Explain home health/home care services to patients, families, and proxies (POAs), aiding in the transition from hospital or rehab to home.

Strategic Planning: Analyze the market, monitor the competitive landscape, and track performance metrics in a CRM to meet, or exceed, referral targets.

Collaboration: Work with clinical teams to ensure seamless service initiation and, in some cases, assist with assessing patients for appropriate care levels.

Required Qualifications & Skills

Book of Business: You must have connections and a "book of business" in St. Louis metropolitan area, that will be useful in getting patient referrals to increase census.
Experience: Previous experience in home health, hospice or home care marketing, business development, or clinical outreach ( 2–3+ years), with a proven track record of generating referrals.
Communication: Excellent verbal and written communication, interpersonal, and presentation skills.

Knowledge: Strong understanding of home health, hospice, and/or personal care services.
Attributes: Self-motivated, proactive, organized, and results-focused.
Requirements: A valid driver's license and reliable transportation required for frequent travel.
